Agency Description:

Overview:  Livingston Memorial Visiting Nurse Association is the oldest non-profit home health and hospice agency in Ventura County. Making house calls since 1947, Livingston provides comprehensive and compassionate home health and hospice services to all regardless of the ability to pay.  For over 65 years, Livingston has provided programs and services that make a difference in the lives of the individuals and families. Livingston is more than just care in the home, for their guiding principle is to serve patients, families and the community with dedication and the commitment to provide health care for all. Services include:

Home Health:   Includes a wide range of skilled nursing, therapy, social work, and home health aide services on an intermittent basis in the patient’s home. Many services that once required a hospital stay are now being performed safely and conveniently at home. Home health care is an affordable and comfortable way to meet health care needs and remain at home. Some of the care includes Nursing, Intravenous Therapy, Wound Care, Cardiac Care, Rehabilitation, Telemedicine, and many other services.

Hospice:  A special program that cares for people with a limited life expectancy. Care is provided wherever the patient resides, whether at home, in a residential care facility, or in a skilled nursing facility. The Hospice Team works together to provide comfort care to meet the physical, emotional and spiritual needs of both the patient and their family and focuses on living and the quality of life. Specially trained volunteers, supported by our professional staff, provide emotional support and respite care.

Palliative Care: Specialized holistic care that focuses on treating the pain and symptoms of a serious illness while supporting patients as they explore their goals for future care. 

Personal Assistance Care:  Customized care to preserve independence and quality of life for those needing help at home. Our Care Givers provide personal assistance, such as bathing, dressing, ambulation, meals, medication reminders, light housekeeping, transportation, socialization and companionship.  All staff are Certified Nursing Assistants and Certified Home Health Aides. 



Rehabilitation: Livingston's rehabilitation team coordinates specialized therapy services to maximize the patient's potential and independence. Our orthopedic case management program is appropriate for patients receiving total hip and knee replacement surgery. Therapists provide teaching, in-home safety evaluation and management from the preoperative visit through hospitalization. The care then continues through home care and transitions to outpatient therapy. Rehabilitation services also coordinate care to assist patients with head, neck and spinal injuries, cerebral vascular accidents, and other conditions requiring extensive rehabilitation.

Telemedicine: An innovative supplemental program that monitors patient status by utilizing several methods. RN scheduled telephone calls to patients assesse overall health status. The nurse may review medications, track vital signs and intervene on various health issues. Another method is utilizing state of the art technology that transmits patient information electronically. Livingston's Telemedicine program promotes patient self-management, keeps the physician informed and helps prevent unnecessary emergency room visits and/or hospitalization.

Community Services: Livingston provides community outreach, professional health education classes, community presentations, bereavement support and additional support groups, all free of charge.
